NOTICE: Upon placing your order for book production with FHP it is assumed that all layout, pagination,               spelling, grammar, punctuation, placement of images, etc. are in order. As a courtesy I am providing you a digital proof of your book, including its cover, for your approval. Your signature will indicate that approval. Please note that you assume all responsibility for any legal, ethical and copyright compliance. Printing of your project will begin when payment and this signed document  are received by FHP. If, after printing has begun a rerun is necessary due to overlooked errors or omissions, the customer assumes responsibility for any additional charges.

          ADDITIONAL PROOFS: I  understand that I can have as many "digital proofs" as necessary. I agree that                each proof beyond two will be a $50 charge.

            I agree that I have placed an order with Family Heritage Publishers. I will receive by email                 an invoice for  payment. Pre-payment is required to start production.  If I cancel my order for any reason a 30% cancellation fee will apply.

Thanks for your signature. Once payment is received your book will be sent to production.
